Evolving from the Diesel Technology Forum in October 2023, its mission is to champion energy conservation, environmental stewardship, and a comprehensive approach to reducing carbon and other emissions. The organization demonstrates how internal combustion engines contribute positively to sustainability, global energy, climate, economic, and environmental goals. Headquartered in Frederick, MD, United States, the Forum aims to be the leading voice for internal combustion engines and to positively influence public and policymaker opinions.

The Forum's activities include public education, outreach, research, and collaboration with a wide range of stakeholders, including government, industry, environmental, and community groups. It focuses on various market segments where internal combustion engines are critical, such as rail, marine, construction, energy, transportation, agriculture, mining, and freight transport. The organization's scope encompasses advanced diesel technology systems, engine components, vehicles, equipment, and a diverse array of fuels, including petroleum-based, renewable, biobased, hydrogen, and eFuels, as well as emissions control technologies.

Under the leadership of Executive Director Allen Schaeffer, the Engine Technology Forum released its 2026 Industry Outlook in January 2026, underscoring the vital role of engine technology in sustaining the economy and achieving national objectives, including advancements in AI and energy resources. In February 2024, the Forum provided testimony opposing certain provisions of the "Agriculture Equipment Right to Repair Act," citing concerns that they could jeopardize federal Clean Air Act guidelines. The organization is currently positioned as a key advocate for advanced engines, highlighting their contributions to economic growth, environmental sustainability, and a sustainable future through decarbonization efforts and the integration of renewable fuels, advocating for a balanced approach that includes internal combustion engines alongside zero-emission technologies.
