Instead, the Connext Global 2026 AI Oversight Survey Report highlights that reliability is a collaborative effort. A striking 70% of users say AI is only dependable when paired with human review, split evenly between those requiring a light review and those needing dedicated human oversight. These findings challenge the prevailing narrative of AI as a replacement for human labor, recasting it as a powerful but fallible tool that demands a robust human safety net to be effective and safe.
The Reality of AI in the Modern Workplace
The survey, which polled 1,000 U.S. adults who use AI in their daily work, paints a picture of a workforce that is deeply engaged with the technology but also keenly aware of its limitations. Far from a “set it and forget it” solution, AI in its current form requires constant vigilance. More than four in five users (82%) report that AI tools need their attention either “sometimes” or “almost every time” they are used.
This need for supervision is not expected to diminish. In fact, nearly two-thirds of respondents (64%) believe the necessity for human review of AI-generated work will increase in the near future. This sentiment underscores a growing understanding that as AI tools are applied to more complex tasks, the potential for error and the need for human judgment also grow.
The concept of “after work”—the tasks required to make AI output usable—has become standard procedure. Nearly all respondents (96%) say they perform follow-up work on AI-generated content. The most common tasks involve editing or fixing the output (42%) and formal review or approval (34%). This suggests that while AI may accelerate the creation of a first draft, the total workflow now includes a built-in, and often time-consuming, quality assurance step performed by humans.

The time and effort required to correct AI errors can significantly erode, and in some cases eliminate, the anticipated efficiency gains. Only 37% of users say AI gets its output right without fixes most of the time. The majority (63%) find that AI is correct only “sometimes” or even less frequently.
This inaccuracy creates a substantial workload. Nearly half of the respondents (46%) reported that fixing an AI’s mistakes takes about the same amount of time as it would to have done the task manually from the start. More concerning, 11% said that the correction process actually takes more time. For a majority of users, the promise of saved time disappears the moment an error occurs.
The root of these failures often lies in AI’s inability to grasp nuance and context. The most cited issue was that the AI left out important details or context (42%). Other major problems included the AI confidently presenting incorrect information—a phenomenon known as “hallucination”—which was reported by 31% of users, and the AI simply causing extra work to fix or redo (32%). These findings align with broader industry concerns about the financial and operational costs of AI errors, prompting a surge in spending on AI governance and data quality platforms to mitigate these risks.

The survey uncovered alarming statistics in this area, with 60% of respondents saying they have personally been involved in a situation where AI negatively affected an outcome. For 19% of users, this involved an AI making a customer situation worse, directly impacting satisfaction and loyalty.
The financial repercussions are also tangible. More than one in ten users (11%) reported that an AI-driven error led to lost revenue or customer churn. These incidents highlight the immense risk to brand reputation and customer trust when AI is deployed in high-stakes, client-facing scenarios without adequate human safeguards. In these moments, the lack of human empathy, contextual understanding, and nuanced judgment can turn a routine interaction into a brand-damaging disaster.
This reality has reinforced the importance of human-in-the-loop models, particularly in customer service and other sensitive functions. While AI can handle repetitive queries and data retrieval, industry experts stress that human agents remain essential for managing complex issues, handling emotional conversations, and providing the ethical and empathetic oversight that builds lasting customer relationships.
